Champlain Geriatric Services is an established community eldercare service operated by Jean McCandless. Jean is a Geriatric Social Worker with more than 30 years experience working with adults and families. She holds a Master’s Degree in Social Work from the University of Chicago and is accredited by both the National Association of Professional Geriatric Care Managers and National Association of Social Workers. Jean is a Licensed Clinical Social Worker (LiCSW) and has a Certificate in Gerontology from the University of Vermont.
A commitment to individualized, person-centered care is an integral part of all Jean’s work. In order to achieve this goal, she collaborates with many other community providers to deliver the high quality services that families need for their elders, whether they live in their own home, an assisted living program, or a nursing home. Elders or their families often seek help during major transitions and unexpected crises when difficult life decisions must be made. Jean specializes in working with elders whose family members live at a distance or who do not have family members available. She has extensive experience working with family lawyers, trust officers, physicians, caregiving agencies, and community services.
Jean’s private practice office has been located in Chittenden County since 1991. Much of her work occurs in people’s homes or in community and residential settings.
